Household Survey Help Request Form Collection Notice

This Collection Notice explains the collection of personal information by the Australian Bureau of Statistics through the Household Survey Help Request Form (HSHRF). The HSHRF is a voluntary form that aims to provide you with an additional way of contacting the ABS if you have questions about the household survey you have been selected for.

In order to contact you about your survey enquiry we will collect the following personal information:

• your telephone number.

We may also collect the following personal information if you choose to provide it:

• names
• home address.
