An improved method to calculate age standardised rates and relative standard errors (RSE) has been introduced which has resulted in small changes to the age standardised data presented in Table 1. A processing error has been corrected in Table 3.1. These changes had no significant impact on the interpretation of the data.

In addition, age standardised rates have been removed from Table 2. Table 8 of the original publication has been removed.
