What do I need to access the form?

To access the form, you need a Unit Identifier and a Passcode.

The Unit Identifier and the Passcode can be found on the letter sent to you by the ABS.

The form is designed to work successfully with the most common Internet browsers, providing they support 128-bit SSL encryption. If your browser does not support 128-bit SSL encryption you will not be able to complete the form online. For information on how to enable 128-bit SSL encryption click on the link to Technical Help.

What if I lose the Unit Identifier?

It is important that the Unit Identifier provided to you is used to login and complete the form.
