2006 Survey of Motor Vehicle Use Sample Questionnaires

Please note: These sample questionnaires are provided for research and background information only. People who have been asked to complete a survey questionnaire and who may have misplaced their original questionnaire are asked to ring 1800 735 060 to request a new hardcopy be mailed out. Do not use the sample questionnaires supplied here.



The type of questionnaire dispatched to the owner of a vehicle selected in the survey is determined by the configuration of the vehicle's body and may not necessarily reflect its use (eg, by their nature utilities are treated as freight vehicles, however they may not necessarily carry any freight).

Passenger Vehicle Pre-Advice (PV-1) [PDF 63kb]
    This questionnaire is sent to selected passenger vehicle owners at the beginning of the survey quarter to collect preliminary details such as an initial odometer reading and to confirm some vehicle specifications and ownership details.
Freight Vehicle Pre-Advice (FV-1) [PDF 64kb]
    This questionnaire is sent to selected freight vehicle owners at the beginning of the survey quarter to collect preliminary details such as an initial odometer reading and to confirm some vehicle specifications and ownership details.
Bus Vehicle Pre-Advice (BUS-1) [PDF 62kb]
    This questionnaire is sent to selected bus owners at the beginning of the survey quarter to collect preliminary details such as an initial odometer reading and to confirm some vehicle specifications and ownership details.


Passenger Vehicle End of Quarter (PV-2) [PDF 71kb]
    This questionnaire is sent to the selected passenger vehicle owners at the end of the survey quarter to collect vehicle useage information for that quarter.
Freight Vehicle End of Quarter (FV-2) [PDF 80kb]
    This questionnaire is sent to the selected freight vehicle owners at the end of the survey quarter to collect vehicle useage information for that quarter.

Bus Vehicle End of Quarter (BUS-2) [PDF 72kb]
    This questionnaire is sent to the selected bus owners at the end of the survey quarter to collect vehicle useage information for that quarter.
