INTRODUCTION

This page contains information to help health establishments that are included in the 2015 Survey of Disability, Ageing and Carers. If your question about the 2015 Survey of Disability, Ageing and Carers - Health Establishment Component is not answered on this page or in the information provided on your form, please contact the ABS on 1800 221 077 (freecall excluding mobile phones).

ABOUT THE SURVEY

The Survey of Disability, Ageing and Carers - Health Establishment Component collects information about the health conditions of long-term occupants in health care and aged care establishments. The Survey of Disability, Ageing and Carers also collects information from people living in households, which is later combined with the information collected from health establishments to ensure the survey represents a comprehensive picture of disability in Australia. The survey is conducted 3 yearly. Summary data from the last survey in 2012 can be accessed in Disability, Ageing and Carers, Australia: Summary of Findings, 2012 (cat. no. 4430.0).

The results from the overall survey are used extensively by government and non-government agencies, researchers, academics and health professionals to:

  • plan and provide services for people with disabilities;
  • understand the needs of an ageing population in Australia;
  • plan the balance between providing formal care and supporting informal care for people in need.

Information is collected from health establishments in 2 stages. For the first stage, all known Australian businesses which may provide accommodation to support long-term care are sent a Contact Information Form. Details from the form are used to determine whether an establishment is eligible for inclusion in Stage 2 of the survey.

Only a sample of in-scope establishments will be selected for Stage 2. An establishment included in Stage 2 will be asked to complete a questionnaire for a selection of long-term residents within their facility.

STAGE 1 - CONTACT INFORMATION FORM

How is information collected?

Stage 1 of the Health Establishment Component of the Survey of Disability, Ageing and Carers uses an online form to collect information. If asked to participate in Stage 1, your establishment will receive a letter providing you with login details and a password. To access your online form, enter the following address into your web browser (i.e. enter into the address bar not into a search engine) http://abs.gov.au/survey. To login, use the 'User Identifier' and 'Password' details that are included in your letter from the ABS. If you have misplaced your letter and login details, please call 1800 221 077 to request a new password or to provide the requested information over the phone.

What information will I be asked to provide?

The Contact Information Form asks health establishments to supply details of an appropriate contact person who is able to provide information on the nature of the health establishment including its main function and the number of residents living in cared accommodation.

How do I submit the web form?

To submit your completed form to the ABS, click the 'Submit' button at the end of the online form. Once you submit the form you will no longer be able to view or update it. Alternatively, you can provide the information to the ABS over the phone.


STAGE 2 - ESTABLISHMENT COMPONENT QUESTIONNAIRE

How is information collected?

Stage 2 of the Health Establishment Component of the Survey of Disability, Ageing and Carers uses a paper form to collect information. Your establishment will receive a letter from the ABS if you are required to participate in Stage 2 of the survey. You will also receive a Selection Form which provides details on how to select the occupants of your establishment to include in the survey, as well as a number of Establishment Component Questionnaires. You will be asked to fill in an Establishment Component Questionnaire for a sample of long-term occupants in your establishment. These will then need to be posted back to the ABS using the reply paid envelope provided.

What information will I be asked to provide?

If your establishment is selected, the person identified through the Contact Information Form will be sent questionnaires and instructions on how to select a sample of residents. To complete the questionnaires, the contact person must be able to provide information on the long-term health conditions of those living in the establishment. This includes information on personal care tasks, such as dressing and eating, and on the use of technical aids to perform everyday tasks. The contact person will not be asked to provide the names of the residents they have selected.

How do I submit the paper forms?

Please post the completed Establishment Component Questionnaires and Selection Form back to the ABS using the mailing address shown on the forms. Reply paid envelopes will be provided.


PARTICIPATION

Do I have to participate?

At this time you are being requested to answer the questions, but if the Australian Statistician directs you in writing to provide the information you are legally obliged to do so. The answers you provide will be treated confidentially. The ABS is required by the Census and Statistics Act 1905 to maintain the secrecy of all information provided to it. No information will be released in a way that would enable an individual or household or organisation to be identified.

What if I cannot complete the survey by the due date?

As your information is important to the ABS, we are happy to discuss an extension to the original due date. To request an extension please contact the ABS on 1800 221 077.

We do not provide long term cared accommodation, why has our establishment been asked to complete the Stage 1 Contact Information Form?

The list of establishments included in Stage 1 of the survey has been generated from a variety of government and non-government sources. These sources often contain insufficient detail to determine the functions of the establishment. Stage 1 seeks to ensure that only eligible establishments are included in Stage 2.
