      Prompt card 5 was shown which included "MIGRAINE" among a list of 41 conditions.       		Migraine was not specifically prompted for in any questions. However, respondents were free to report "migraine" in response to Q448: "DO YOU HAVE ANY (OTHER) CONDITIONS THAT HAVE LASTED OR ARE EXPECTED TO LAST FOR SIX MONTHS OR MORE, FOR EXAMPLE:" Prompt card 'H' was shown, including a list of 23 conditions.
